In patients with diabetic neuropathy, ulcers are located primarily at sites of repeated trauma and usually affect the sole surface of the foot near the heads of the metatarsal bones, the heel and toes. Neuropathic ulcers are usually not painful.
In addition, as a result of neuroarthropathic disorders, the foot undergoes a characteristic deformation – it becomes more grooved, the toes become hammer-shaped, the gait changes, and calluses or corns form in places where the foot is incorrectly supported, which may later lead to hard-to-heal wounds and ulcers.
In ischaemic diabetic foot, chronic tissue hypoxia causes the foot to begin to die – swellings, ulcers, necrosis and cracks form, nails deform, soft tissues disappear and the entire foot begins to turn blue.
